Mosquitofish (Gambusia) - Request Form

Mosquitofish (Gambusia affinis) can be a good option, in certain situations, to provide an ongoing, biological control of immature mosquitoes (larvae and pupae).  If you are interested in requesting mosquito fish, please fill out the following form.

** The Mosquito Control District has a special permit from the Washington State Department of Fish and Wildlife (WDFW) to transport and plant mosquitofish into isolated water bodies. By providing your electronic signature below, you are acknowledging that you will NOT transport these fish from your water site to any other water site, nor share these fish with any other person or entity. Additionally, your e-signature verifies that you are the legal owner of the property that has the water site in question.
